AYUSH medicos don role of main docs

Vadodara/Savli Two years ago, the National Rural Health Mission (NRHM) had devised a plan by which Ayurveda, Yoga, Unani, Siddha and Homoeopathy (AYUSH) doctors were to work as helpers to MBBS doctors at Primary Health Centres (PHCs) in villages. But because of the shortage of MBBS doctors in rural areas, AYUSH doctors are forced to function as main doctors at these centres. As per the recent data, over 428 ayurvedic doctors and 228 homeopathic doctors are recruited against 647 MBBS doctors at the state’s 1,080 PHCs under the NRHM’s AYUSH project.

At 76 PHCs in Vadodara district, over 43 AYUSH doctors are recruited against 51 MBBS doctors. The inability to post MBBS doctors at rural centres has made the AYUSH doctors take charge of the entire unit.

As per the NRHM guidelines, AYUSH doctors were recruited for

supervising the national healthcare programmes in villages and give preventive medicines.

They were to dispense medicines to control communicable diseases and vector and water borne diseases, supervise tuberculosis cases and work for decreasing maternal mortality and infant mortality rates. Instead, now AYUSH doctors are required to handle not only routine treatment but also complicated and emergency cases at the PHCs.

While the PHC at Pandu village in Savli does not have any MBBS doctor for past two years, over 525 deliveries were carried out at the PHC by an AYUSH doctor.

“The nearest health care unit having an MBBS doctor is the Community Health Centre (CHC) at Savli village which is very far from here. With the pregnant women coming at the last moment, immediate deliveries are to be conducted,” said Jyotsna Jadav, an AYUSH worker in Savli taluka.

She said though critical cases are sent to the CHC, in emergency complicated cases have also been handled here as well,” said Jadav.

“A large number of accidents take place on this stretch of road at Desar and the victims have to be invariably taken to Savli CHC as no MBBS doctor is available anywhere else. Many a times, victims die on the route,” said Bharat Chauhan a resident of Desar village.

Although AYUSH doctors are not allowed to take medico-legal cases, besides giving ayurvedic and homeopathic medicines to patients, these doctors often have to take up other modes of medication in emergencies.

“I am hardly able to carry out the field work that I am required to do. There are at least 30 to 40 patients at the centre daily. Even though I keep myself restricted to ayurvedic medicines, in emergencies I am forced to carry out duties of an MBBS doctor,” said an ayurvedic (AYUSH) doctor at a village in Savli taluka.

Vadodara Chief District Health Officer, Dr Bhagwat Itare, said, “MBBS doctors do not want to take up rural postings and hence their posts at PHCs remain vacant. There are many PHCs without MBBS doctors but AYUSH doctors are helpful in taking up preventive measures and handle emergency cases.”

the only Doctors available on the earth that can provide widest spectrum of healthcare (if trained in the areas left to be trained) are AYUSH Doctors, Allopathy may be better utilised and advanced in the country where AYUSH is available to tie up with! and not with Allopathy alone at all..!

Govt.of india has well strategic planning unit.They plan efficiently but not implemented properly.In this context the idea of BRMS degree came when Rural healthcare is concerned Govt. trying to train a separate cader of professionals of health care but the idea to impart training to AYUSH Doctor (BHMS & BAMS)in Allopathy and absob them in the system was not thought.A homoeopathic or Ayurvedic physician can better manage the rural health care in PHCS & CHCS if trained for a short period.Govt.had planned an implimented the EMOC and ANAESTHEIA training of 6 months for MBBS doctors in NRHM scheme, it is well and appriciated.Govt. should train the AYUSH doctors in ALLOPATHY so a pool of doctors can be availed for the underprivleged rural mass &the doctor shortage can be fullfilled.The govt of Assam&Chatishgarh started this policy.To day most of the people are attracted to Homoeopathy&Ayurveda so mainstreaming of these systems will happen & we will get an integrated&comprenhesive care.

the academic career of bams doctors are very good.and it is also not so easy to get admission in bams course.now a days the syllabus of bams course is comprises of both allopathy and ayurveda.and they are also welltrained in emergency management.that is why students are interested in studying bams now a days.if any restriction in prescribing allopathy medicine then govt should had to change syllabus of bams course before.now a days many bams docs doing md course in surgery,gynacology,anasthesia and giving good service to people.so why should they are neglected many times.but it is real fact that bams docs are also know properly how to use allo medicine.

Govt is searching new mbbs doctors to recruit in rural india but the lack of mbbs doctors in the country creats a panic among the government and rural people.so the govt. decided to create a cadre of rural doctors ,named the BRMS degree which is of three and half years duration.But the main aim of govt. should be aware of the people of AYUSH stream who are studied for five and half years so govt. should think to train those AYUSH doctors in allopathy and emergency care so that they can be better utilised in rural india and dont waste time in creating the BRMS DEGREE and donot give the apple to the rural people after four years.

Now a days BAMS docs are also well trained,they can well manage PHCs, if proper training is being given.if MBBS docs think that only they should be allowed to work over ther then why they are busy in earning substantial money in URBUN cities, why huge quantity of MBBS docs are lacking in rural areas.why they can't show there dexerity in the field of pharma in rural area.when there were no hospital facility since there ayurveda ia existing so we should not forget our legecy,at least we should respect it.

Rural people have already been deprived from basic ammenites of life from ages.WHY should be they deprived from modern system of medicine and instead subjected to experimentation and treated as guinea pigs? Instead of messing up the things why cant docs/veds practise the system of medicine in which they have been trained over a span of 5 or 6 years? Further more there is no dearth of MBBS doctors available for service IN OUR COUNTRY.but there appear hardly any will of Govt to appoint them in time,because PSC interviews for docs are never held on regular basis in any of our states not even a hundred per year on an average.Whereas on an average most states in india produces 400-500 MBBS doctors every year.WHERE THEY ALL GO???????GOVT SHOULD THINK ABOUT ALL THIS MESS...INSTEAD COPELLING MBBS docs TO LOOK FOR JOBS ELSEWHERE.Infact because of lack of proper manpower management the outside countries are taking the benefits.30-50% of docs in usa, uk so many other counties are from india.Y????

AND THE DEGREE OF SAFETY PROFILE IS A GREAT CONCERN.SAFETY PROFILE WHICH COMES WITH THE INFORMATION PAMPLETS WITH THE DRUG IS WHAT HAS BEEN DETECTED UNDER CONTROLLED TRIALS.AND CAN DETECT THE SIDE EFFECTS OCCURING AT AN INCIDENCE OF 1:250. MOST RARE SIDE EFFECTS ARE TO BE DETECTED DURING POST MARKETING SUVILANCE.i e. WHAT A RESEARCHER WILL DO.ALLOPATH IS NOT SIMPLY A DOCTOR BUT A RESEARCH SCHOLER AS WELL. The role of a doctor is to treat a patient what ever the systen he uses,but ehe thing is that when a law comes that makes a bit difficult to handle the patients perfectly.AYUSH doctors are qualified in all the medical subjects excluding the ALLOPATHIC PHARMACOLOGY so,if they are trained in that subject and also in EMERGENSY CARE in a medical college in short term basis they also can efficiently manage the day to day treatments in a rural hospital and also the poor rural people can get the benifits of their services.They can also actively manage the NATIONAL HEALTH PROGRAMMES and other admnistrative activities.so,to meet the shortage of MBBS doctors they are very desirable in health care delivary system.it is high time to train the AYUSH DOCTORS and examine them in the subjects,give them liscence to practice ALLOPATHY so that they can be actively engaged in betternent of health care of rural india.

maharashtra government has given legal rights to doctors of BAMS faculty in maharashtra to practise allopathic medicine to the extent they are taught in there college. these step has improved the health status in maharashtra as compared to any other states in India. As the syllabus covered in BAMS colleges comprises of more than 50 % allopath, and in current practice it is seen that people of BAMS are serving 90% people in villages of which only 10 %are covered by the MBBS People.

THIS IS IN RESPONSE TO THE STORY-"AYUSH MEDICOS DON ROLE OF MAIN DOCS"-THIS ARTICLE CLEARLY SHOWS THAT EVEN THE AYUSH DOCTORS ARE CAPABLE OF HANDLING THE EMERGENCIES IN RURAL AREAS WHERE THE MBBS DOCTORS ARE NOT AVILABLE.IT BECOMES ILLEGAL WHEN A PRIVATE AYUSH DOCTOR PRESCRIBES ALLOPATHIC MEDICINES AND LEGAL WHEN A GOVT.DOCTORS DOES THE SAME.ISN'T IT RIDICULOUS.IT IS HIGH TIME THAT TO TRAIN ALLOPATHIC PHARMACOLOGY TO ALL AYUSH DOCTORS AND ALLOW THEM TO USE ALLOPATHIC DRUGS, IF NOT REGULARLY, ATLEAST IN EMERGENCY. MOST OF THE AYUSH DOCTORS ARE PRACTICING IN RURAL AREA WITHOUT SUCH TRAINING IN ALLOPATHIC PHARMOCOLOGY WHICH MAY SUFFER THE PATIENTS DUE TO PARTIAL KNOWLEDGE OF THE PHYSICIAN.IF YOU TRAIN THEM ALLOPATHIC PHARMACY AND CONDUCT EXAMINATION IN THE SUBJECT,THAT WOULD DO WONDERFUL BENEFITS TO RURAL PATIENTS.GOVERNMENT SHOULD THINK IT OVER.
